Fragmented entries scattered across a 12-hour shift. The assumption that ambient AI proven for physician narratives transfers cleanly to nursing workflow automation is untested.

There's also a business model problem. Physician notes generate billable revenue under fee-for-service reimbursement. That creates a direct ROI case for hospitals. Nursing documentation doesn't generate revenue — it serves compliance, coordination, and liability protection. EHR burden reduction for nurses has to be justified through indirect savings: reduced overtime, lower turnover, and fewer errors. That's a harder sell to a CFO.
